David Larsen Hafen, 86, passed away peacefully on September 11, 2025, surrounded by his loving wife and children after a brief battle with injuries following a recent car accident.

Born October 23, 1938, in Mt. Pleasant, UT, to Jay and Ruby Hafen, he was the youngest of three children. He spent his youth working on his family farm, shepherding sheep in the mountains during the summer, and was a talented high school athlete. He attended Snow College in Ephraim, UT, before beginning his career in the insurance industry. He also served in the Utah National Guard.

He married his high school sweetheart, Carolyn Blain, and they were blessed to celebrate their 68th anniversary together, encircled by family, four days before his passing. Together, they built a life filled with love, service, and faith. Family was always their highest priority, and they were blessed to welcome eight children into their home along with twenty-three grandchildren and twelve great-grandchildren.

Dave built a successful career in the insurance industry in the Walla Walla community, spending the majority of his career as an insurance broker at McDonald Zaring Insurance (MZI) in Walla Walla.

Faith was always at the center of Dave’s life. He was an active member of The Church of Jesus Christ of Latter-day Saints, serving in numerous capacities and leadership positions. He and Carolyn served a church mission on the Navajo Nation in Window Rock, AZ. He was privileged to be chosen as the first stake president of the newly formed Walla Walla Stake. Countless individuals over the years were blessed by his counsel, compassion, and charity. Anyone ever interacting with Dave never left their encounter without being warmly greeted and hugged. He will be remembered for his warmth, humility, and the way he made everyone feel welcome in his presence. Truly, he was a friend and servant to all.

Dave is survived by his wife, Carolyn; daughters, Cathy (Phil) of Fall City, WA, Kimberly (Mike) of Queen Creek, AZ, and Nikol of Salt Lake City, UT; and sons, Robert (Lura), Brad (Deirdre), and Ky (Tambri) all of Walla Walla; twenty-three grandchildren and twelve great grandchildren. He was preceded in death by his parents, his sister Mary, his brother Bill, two dear children, Mark and Allison, and his granddaughter Tia.

A Celebration of Life will be held at 11:00 AM on his birthday, Thursday, October 23, 2025, at The Church of Jesus Christ of Latter-day Saints on 1821 S Second Ave in Walla Walla. He will be interred at the Mt. Pleasant City Cemetery in Mt. Pleasant, UT Saturday, April 11, 2026 at 11:00 a.m.. In lieu of flowers, the family requests donations be made to the Church General Missionary Fund and the Navajo United Way in Window Rock.

The family would like to thank the caring staff at Providence St. Mary Medical Center for all their professional care and personal touches. They also appreciate the outpouring of love from family, friends, and the community.

LEE ALBERT PHILLIPS JR. ~~~A LIFELONG FRIEND AND FORMER BOARD MEMBER OF MT. PLEASANT HISTORICAL ASSOCIATION

 

 Lee Albert Phillips Jr., 81, of St. George, Utah, passed away peacefully on January 3, 2026, after a battle with Alzheimer's. Born in Mount Pleasant, Utah on December 2, 1944, Lee lived a life defined by service, leadership, and an unwavering devotion to his family and community.

Lee was the beloved son of Lee Albert Phillips Sr. and Glenna Rosina Hafen Phillips. He is survived by his cherished wife of fifty-seven years, Carol Lynn Herrin Phillips, who stood beside him through all of life's journey. Together, they built a beautiful family that was the center of Lee's world.

He was a proud father to five children: Lori Phillips (Darren), Jeffery Phillips (Becky), Tami Finau (Brett), Steven Phillips, and Travis Phillips (Aundrea). Lee's legacy continues through his18 grandchildren: Kameron Simonson (Ashlee), Meghan Peacock (Tyler), Braydon Simonson, Jakob Simonson (Dasya), Lukas Simonson, Alex Phillips, Robert Phillips, Krista Phillips, Lexi Phillips, Logan Phillips, Kilisi Finau, Keila Finau, Maili Finau, Keoni Finau, and Carson Phillips. He was also blessed with seven great-grandchildren: Hendrix Simonson, Axel Simonson, Madison Peacock, Miles Peacock, and Nova Simonson, who brought immense joy to his later years.

Lee is preceded in death by his parents, his sister Lois Monsen, and his grandson Skyler Phillips. He is survived by his sister Joan Markosian, with whom he shared many cherished memories.

Lee dedicated his professional life to banking, building a distinguished career at First Security Bank and later Wells Fargo Bank. His business acumen and commitment to excellence earned him respect throughout his career and the trust of countless colleagues and clients.

Beyond his professional achievements, Lee was a servant leader who generously gave of his time and talents to his community. He served as President of the Chamber of Commerce, President of the Sun Shiners, and President of the Rotary Club, where he was honored as a Paul Harris Fellow. He also served as Treasurer of the Salvation Army, demonstrating his commitment to helping those in need. His faith was central to his life, and he held many leadership positions in The Church of Jesus Christ of Latter-day Saints, including serving a mission in the Eastern Atlantic States from 1964 to 1966.

Lee graduated from Brigham Young University in 1971 with a degree in Sociology. As a proud alumnus, he remained a devoted fan throughout his life, regularly attending and watching BYU football and basketball games.

Those who knew Lee will remember him for his warm spirit and the smile that would light up any room. He was an avid golfer who enjoyed time on the course, but his true passions were found closer to home. Lee loved singing and in his earlier years, playing the guitar. He took great pride in maintaining his home and yards and found great satisfaction in mowing the lawn and making sure his lawn was the greenest one on the block.

One of Lee's most endearing qualities was his playful nature with his grandchildren. He would delight them with his Donald Duck voice, creating memories that will be cherished for generations. His ability to bring laughter and joy to his family was one of his greatest gifts.

Lee Albert Phillips Jr. was a man of integrity, kindness, and boundless love. His legacy lives on through the countless lives he touched, the community he served, and the family he loved so dearly. He will be deeply missed by all who had the privilege of knowing him.

A celebration of Lee's life will be held on Saturday, January 10, 2026, at Metcalf Bonzo Mortuary (288 W. St. George Blvd, St. George, Utah). Friends and family are invited to a viewing from 1:00 to 2:00 p.m., followed by a sharing of memories from 2:00 to 3:00 p.m.

Peter Leon Hafen, 87, returned to his Heavenly Father on June 11, 2025.

Born on June 2, 1938, Peter lived a remarkable life filled with adventure, hard work, and talents few could match. A master welder, mechanic, carpenter, & blacksmith, Peter was known to be able to fix or build anything. If it was broken, he could make it work again—usually better than before.

In high school, Peter stood out as an athlete, known for his incredible speed and toughness in any sport he played. When a series of concussions put an end to his football dreams, Pete did what any determined young man would do—he started riding bulls in rodeos across the Sanpete Valley.

Music was another of Peter’s many gifts. He could play just about anything—including the saxophone, trumpet, piano, guitar, and banjo. He and his brother Donald—his lifelong sidekick and partner in crime—performed in a band called The 7 Hits & A Miss. They played for dances all over central and eastern Utah.

Peter was also a licensed barber who once cut hair in the legendary Hotel Utah. Later, he opened barber shops in both Provo and his hometown of Mount Pleasant. And if that wasn’t enough, he was also a talented baker—known for beautifully decorated cakes and delicious homemade pies.

On September 9, 1966, Peter married his sweetheart, Kathy Rigby Hafen. They were later sealed in the Manti Temple on August 9, 1977. Together, they raised three children—Michael, Laura, and Jon—and were blessed with 10 grandchildren and 2 great-grandchildren.

There’s so much more we could say about Peter Hafen. He was full of wisdom, loved his family unconditionally, & was gifted with a sense of humor that came with great advice often shared with us. To this end we finish with one of his favorite lines: “Never ride a bull backwards.” There’s a story behind that—and plenty of others that we’ll hold close to our hearts until we meet again.

Funeral services for Peter will include a viewing on Tuesday, June 17, 2025, from 6:00 to 7:00 PM at Rasmussen Mortuary, 96 N 100 W, Mount Pleasant.

A second viewing will be held Wednesday morning from 9:30 to 10:30 AM, followed by funeral services at 11:00 AM at the Mount Pleasant LDS North Stake Center, 461 N 300 W, Mount Pleasant. Online condolences at rasmussenmortuary.com

Lois Phillips Monsen was born in Mt Pleasant Utah on July 7, 1935 to Glenna Rozina Phillips and Lee Albert Phillips.

From the beginning Lois was a protector and nurturer, these traits were most apparent when her parents brought home her little sister Joan and then her baby brother Lee.

Growing up in Mt. Pleasant, Lois met her husband Stephen Monsen in elementary school, they later became high school sweethearts and went on to get married and sealed in the Manti Temple on June 29, 1956.
One year later they welcomed their first son Stephen Craig Monsen and just one year after that, their second son, Robert Tod Monsen was born. And Finally, the Monsen family grew to 5 with the birth of their daughter Stephanie Monsen Anderson.

The first few years of married life were a beautiful blur as Lois was busy being a nurturer and protector to her children. She kept everything running while her husband attended school. Lois was committed to providing a great life for her family, sometimes working outside the home to help give her children the best opportunities. Lois was an immaculate house keeper and an incredible cook.
As Lois became a grandmother to 19 grandkids and 38 great grandchildren, she continued to be a nurturer and protector to her ever growing family.

Lois was a devout member of the Church of Jesus Christ of Latter-Day Saints. The gospel was always central in her life as she spent time in many church callings. Some of her favorites included working with the Young Women and being an ordinance worker for the Provo Temple. Lois and Stephen accepted the opportunity to serve a mission in the Mormon Trail Center for the Church of Jesus Christ of Latter Day Saints in the at Historic Winter Quarters in the Nebraska Omaha Mission from 2012-2013.
Lois was also a member of the National Society of the Daughters of Utah Pioneers.

Lois was a true example of Christs love. She always had a smile, a hug and an I Love You for everyone. Her listening ear, laughter, quick wit and ability to give the best advice will be greatly missed.

On March 13, 2024 Lois was reunited with her Heavenly Father and her beloved Stephen.
Lois leaves behind her sister Joan (Lou) Markosian, her brother Lee (Carol) Phillips, her 3 children: Craig (Shelly) Monsen, Tod (Robi) Monsen and Stephanie (Jon) Anderson, 17 grandchildren and 38 great grandchildren.

Lois was preceded in death by her parents Glenna Rozina Hafen and Lee Albert Phillips, her husband Stephen Monsen, her son-in-law Jon Anderson, her granddaughter Amy Monsen and her grandson Jessie Monsen.

Services were held at the Mapleton 2nd Ward Meeting house 1050 N 300 W

Allan Leon Beck, 89, beloved husband, father, grandfather and great-grandfather, passed away peacefully on March 14, 2023, at his home in Orem, Utah, surrounded by his family just short of his 90th birthday. Allan was born on April 28, 1933, to Earl Hafen Beck and Ruth Olsen Beck in Mt. Pleasant, Utah. He was one of five children, Earl Duane (passed at birth), Allan, Eldon, Earlene and Ruth Ann.

He grew up in Indianola, Utah, and was always very active and loved spending time outdoors, hunting, fishing, and playing football, basketball and track. He graduated from North Sanpete High School in 1951, and afterward attended the University of Utah in 1952. He served in the Utah National Guard and the U.S. Army Reserve, spending time overseas in Taiwan. Allan was very proud to be a Veteran and had a great love for our nation’s flag.

He married his high school sweetheart Marian Ruth Nielson on June 4, 1953. They were married 25 years and were blessed with four children, Mark Allan, Malinda, Marianne and Matthew.

Allan had a love for learning and could build or fix anything he put his mind to. He built the first workable television in the area at Indianola in 1949. It is now in the Fairview Museum. He installed the first TV in Mt. Pleasant at Herman Beck’s home before TV stations moved their transmitting stations high on the Oquirrh Mountains west of Salt Lake City.

He founded Beck’s TV, a local business in Mt. Pleasant in 1958. His son Mark Allan took over the family business in 1980 and it continues today as a retail home furnishings business on Main Street.

In January 1958, he engineered and installed Sanpete County’s first television translator relay site, located at the base of the mountain east of Mt. Pleasant. This translator station made TV possible for Mt. Pleasant and surrounding towns and is still functioning today.

He held many offices in the Chamber of Commerce, including the president. He was a charter member of JC’s and the Sanpete County Search & Rescue and held several offices in both organizations. He served as Mayor of Mt. Pleasant (1978-1980) and worked hard to make improvements to the city, including the municipal sewer system and settling ponds west of the airport. He also was able to get the airport improved from gravel to a paved landing surface.

On July 23, 1980, he married the love of his life Betty Moulton Harvey at “The Lake” in Indianola, Utah, and increased his family by two children, Craig and Katie Sue. They were later sealed in the Provo Utah LDS Temple on April 28, 1984. They made their home in Orem, Utah.

They loved spending time in Indianola, and created a Hideaway there, constantly improving and caretaking the pond on the mountain (“The Lake”) and considered it their home away from home. Their family, including children and grandchildren, have come to love The Lake and the beautiful surroundings that Allan has been instrumental in creating.

Allan also worked at Wells Distributing and then Ryan Distributing, both in Salt Lake City, as Service Manager of their appliance division, and later worked at Utah Valley Regional Medical Center (Provo) in the Engineering Department, fixing and maintaining all the electronics in the facility. He retired from Intermountain Healthcare in 1996 so that he could spend more time in his favorite place, Indianola.

He is survived by his wife, Betty Moulton Beck and 6 children, Mark Allan (Laura) Beck, Malinda (Scott) Overman, Craig (Carolyn) Harvey, Marianne (Patrick) Davis, Matthew (Becky) Beck, Katie (Eric) Peterson, 24 grandchildren and 60 great-grandchildren.

Funeral services will be held Monday, March 20, 2023, 11:30 a.m. at the LDS chapel on 500 South 600 West Orem, UT. Viewing 9:00-11:00 a.m. prior to the service. Interment will be in the Mount Pleasant City Cemetery. Many thanks to Dignity Home Health & Hospice team, especially Markae, Marisela and Melissa who took great care in assisting Allan in his final days.

OBITUARY

Leo Osman Osborne, beloved father and grandfather, passed away on July 12, 2019 surrounded by his family after a courageous battle with cancer.

Leo was born the third child out of three sons on October 16, 1936 in Spring City, Utah to William Virgus and Athene Osborne. He graduated from North Sanpete High School where he was involved in sports, speech and debate and drama. He also graduated from Snow College and attended BYU. He married Lou Ann Hafen on December 9, 1955 in the Manti Temple. He and Lou Ann raised four children, Lyle, Laura Lee, Lance and Lorette, and lived in Nephi, Utah for 56 years.

Leo was an active member of The Church of Jesus Christ of Latter-day Saints where he served in many callings including scout leader, Elders Quorum presidency, Sunday School teacher and president, and assistant ward clerk for 36 years.

He was also active in the community, helping with little league baseball and football. He enjoyed advising the Juab High School Rodeo Club, but one of his greatest accomplishments and enjoyments was being a member of the Ute Stampede Committee. He served on this committee for 49 years and Leo made life-long friendships with the men and women he served with. Leo worked for the Juab County ASCS office as a manager, and through this employment, he was involved in the National Association of ASCS where he was voted as the vice-president and president of the Utah group. He went on to serve as the secretary-treasurer with the national group of the NASCOE for 13 years and RASCOE for 7 years. Through these groups Leo traveled the country and was well known and respected for his work.

Leo enjoyed the time he spent each year hunting the Spring City mountains with his dad, brothers, sons, grandsons and nephews. He loved his family very much and he did everything he could for each individual to show them how much Grandpa Ozzie loved them. He could be heard at every function his children and grandchildren were involved in, either cheering them on or trying to make his point.

He is survived by his children, Lyle (Jodie) Osborne, Laura Lee (Kevin) Ockey, Lance (Tausha) Osborne, and Lorette Nielsen. He was preceded in death by his loving wife of 63 years, Lou Ann Hafen Osborne, his parents, and his two brothers, Connell and Glen Osborne.

Lou Ann Hafen Osborne Has Passed On

Lou Ann Hafen Osborne, beloved wife, mother, and grandmother, passed away June 20, 2019 at her home surrounded by her family after a courageous battle with cancer.

She was born the third of four children on August 2, 1937 in Mt. Pleasant Utah to Mack and Merdith Hafen. She graduated from North Sanpete High School with honors. On December 9, 1955 she married Leo Osborne in the Manti Temple.

She raised her four children Lyle, Laura Lee, Lance and Lorette in Nephi Utah.

Lou Ann was an active member of The Church of Jesus Christ of Latter-day Saints and served faithfully her whole life in many callings, including President of the Young Women in her ward, Ward Relief Society President, Stake Relief Society President, Visiting Teacher, Chorister, and Choir Director. She developed many friendships while serving in these callings and she loved helping others and sharing her testimony through music. She also developed life-long friendships with those she worked with for 23 years as a school lunch lady.

Lou Ann was very organized, and kept everything around her immaculate. She honestly believed that “Cleanliness was next to Godliness.”

She loved music. Starting at a very young age she sang with her sisters and she continued throughout her life performing with her beautiful soprano voice in many programs and funerals. Many lives have been touched by her wonderful talent.

Lou Ann loved her family. She could always be seen at every function any of her children were involved in. Then she enjoyed even more supporting her grandchildren in everything they did. Every one of her grandchildren loved their “Grandma Honey.”

She is survived by her husband Leo of 58 years, her four children, Lyle, (Jodie) Osborne, Laura Lee (Kevin) Ockey, Lance (Tausha) Osborne and Lorette Nielsen, all of Nephi, two sisters, Shana Stevens of Ohio and Marcia Dansie of California, one brother, Ed Hafen of Idaho, 10 grandchildren and five great grandchildren.

Funeral services will be held Friday, June 28, 2019 at 11:00 am at the Nephi 7th Ward Chapel (222 S 100 E, Nephi, Utah) with a viewing 1 hour prior. Friends and family may call from 6:00-8:00 pm on Thursday, June 27, 2019 at the Anderson Funeral Home, 94 W 300 N, Nephi, Utah.

George Wilford Johansen, 74, passed away February 26, 2016 at his home in Mt. Pleasant, Utah. He was born August 11, 1941 to Dean Niels Johansen and Evelyne Jane Whitman. George married Barbara Ann Graff on April 10, 1965 in Parowan, Utah. They were later sealed for time and all eternity in the LDS Manti Temple.
George worked hard his entire life. He started and ran two successful businesses and a farm. Through those business ventures his children and countless others learned the value of an honest day’s work and its rewards. George always assumed you could perform any given task or find any given location without much direction – we all learned to problem solve on the fly. He was a self-proclaimed doctor and a connoisseur of convenience store fare. George was active in his church and community. He served as a Stake President, a Bishop, a member of the High Council, a counselor in the Single Adult Ward and as a Primary Teacher. He served as a City Councilman and a member of the Planning and Zoning Committee. He had a strong belief that hard work and a backhoe could fix any physical ailment or home repair. He doted on his wife, kids, grandkids and his cows and made sure that all were home and safe each night. He loved living in Sanpete County and could only live without Sanpete County air and water for short periods of time. He was a magnificent teacher, by example. He taught us that music in the car isn’t just there for background noise. It needs to be sung out loud with hand gestures. That if someone is in need you jump in and help – no questions asked. That everyone is a child of God and deserves to be treated as such – God will judge later. Forgive and forget no matter how hurtful another person is being. George was a man with a body and a heart bigger than the average man. He will be missed but his influence will never be gone. He was a wonderful husband, father and grandfather. He loved unconditionally and that instilled a sense of safety and security that allowed all of us to grow and become who we are today.
George was preceded in death by his parents Dean and Evelyne Johansen. He is survived by his wife Barbara Ann Johansen of Mt. Pleasant, his five children: Cindy Lynn Johansen of Mt. Pleasant, Christy C. Johansen of Mt. Pleasant, Dean George (Laura) Johansen of Providence, John Graff Johansen of Mt. Pleasant and Karen Ann (Clint) Sorensen of Heber, his seven grandchildren: Duncan George Johansen, Dawson Peter Johansen, Beckham Dean Johansen, Roman John Johansen, Wrigley Clint Sorensen, Reese Ann Sorensen and Atticus Cole Sorensen, his siblings: Alden (Martha) Johansen and Rodger (Jenni) Johansen.
